Answer a few short questions and we will get back to you within 3 working days. Please do not use this online form for urgent or emergency requests. The submitted forms will only be read during Opening Hours. The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays. They will not be read Monday-Friday between the hours of 1800 - 0900. Please contact 111 during out of hours or 999 if this is an emergency.


Online Services

GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. How to sign-up

Call reception and they will ask you a couple of ID questions and once confirmed they will be able to sign you up for Online Services.

Due to COVID 19 restrictions booking appointments Online is still unavailable.  You can either call the surgery on 01702 463333 and speak to reception to book a telephone appointment with a clinician, or you can visit our Website at www.queenswaysurgery.co.uk and follow the Patient Triage Link and send our clinicians your enquiry.  For Urgent enquiries always use the telephone.  You can still use Online Services for Medication requests, Records requests, Immunisation records and Questionnaires.
